CMHA Senior Intensive Case Management

This program is for Seniors who are experiencing serious and persistent mental health concerns. Seniors having challenges accessing supports due to geography, isolation or changes in informal support system are considered a priority for this service.

    Who can benefit?

    Seniors experiencing serious and persistent mental health concerns which include: depression, mood changes, dementia, responsive behaviours, and/or anxiety.

    What are the major benefits?

    The program makes referrals to, and collaborates with, community partners such as CCAC, Seniors Day Programs, Alzheimer’s Society of Dufferin and local hospitals.
    Education and support to the client and care team, to ensure clients have the necessary services and supports to live successfully in the community and to maintain/improve quality of life.
